What we can do

We can offer all kinds of help to all families. The care we can provide includes:

  • ongoing support for life limiting & life threatening illnesses and complex health care needs
  • follow up visits
  • education and training
  • bereavement support
  • transition to adult services
  • liaise with Tertiary Centres
  • care co-ordination
  • support long-term conditions

Our Aims

We aim to help patients and families in the following ways:

  • by preventing admission or re-admission to hospital. or, by making a child's stay in hospital shorter
  • by offering a high standard of care in the community from Registered Children's Nurses
  • to help, support, teach and give information to children their families, and carers. This helps them become as independent as possible
  • to encourage normality within the family

We work closely with other health professionals and specialist centres. We also liaise with closely with other agencies. This improves communication and helps us to plan care for children and their families
